Clinical Study of Thrombocytopenia in Dengue Fever

INTORODUCTION: Dengue is a mosquito-borne viral disease by Flaviviridae family causing wide spectrum of ranging from subclinical disease, severe flu-like symptoms to dengue associated with bleeding, organ impairment and/or plasma leakage higher risk death when not managed appropriately. WHO classifies as DF/ DHF/DSS earlier, at present (with or without warning signs) and Severe Dengue. The virus may be detected NS1antigen ELISA IgM IgG antibodies. Thrombocytopenia one the laboratory parameter in grading severity, study planned during an epidemic south Karnataka region which would have substantial impact on management reducing morbidity mortality dengue. AIMS AND OBJECTIVES: To correlate signs, parameters different age groups, sex, severity thrombocytopenia management. MATERIAL METHODS: Prospective observational conducted for patients more than 15 years NS1Antigen positive IgM/IgG Platelet count less 1, 50,000 per dl. Detailed clinical history detailed physical examination was recorded prescribed preform subjected NS1Antigen, antibodies, Complete blood count, Random Blood Sugar, Urine analysis, Liver Function tests, Renal Abdominal sonography, ECG conservatively. Data entered Microsoft Office Excel Sheet 2010 analyzed. RESULTS: There are 124 thrombocytopenia, out 76 severe,34 moderate 14 mild ,74(60%) NS1 57(46%) positive,79 males (64%) 45 females (36%), 49%are 15-30 (12% 15-20, 37% 21-30)and 20% 31-40age, commonest presentations along fever Headache 70%, myalgia 68%, nausea vomiting 41%, arthralgia 27%, signs abdominal pain 18%, bleeding manifestations 6%, restlessness 8%, altered sensorium 4%, bradycardia 26%, hypotension 17%, hepatomegaly 11%, abnormal leucopenia 43%, renal function tests 20%, elevated liver enzymes 35% cases88%.There 88% signs,24% 17% circulatory collapse, 14% renal, 8% hepatic 4% nervous involvement. transfusion done only 27%. CONCLUSION: Moderate young adults / sign followed pain, manifestations, restlessness, sensorium, leucopenia, bradycardia, hypotension, polyserositis, tenderness, hepatomegaly, enzymes, hyperbilirubinemia, . We describe 2 patients with autoimmune thrombocytopenic disease who developed classic dengue fever associated with serious bleeding and extremely low platelet counts (1000 cells/mm(3) and 3000 cells/mm(3), respectively). Such patients should be properly advised as to the possibility that common dengue fever may substantially enhance their risk for hemorrhagic complications.
